When requesting a certificate we will need the following information:

1. Name of Person Baptized (required)
2. Date of Birth (required)
3. Place of Birth (required)
4. Date of Baptism: month and year (required)
5. Father's Full Name (required)
6. Mother's Maiden (required)
7. Requested by: First and Last name
8. Address, City, State
9. Phone Number

A $5 dollar non-refundable process fee for each certificate that is being requested. Please allow 1-2 weeks for certificate to be returned once we have received the request.
